The PIMS Accident & Incident Investigation course develops the practical skills required to investigate workplace accidents, incidents and near misses using a structured approach. Effective investigations go beyond identifying what happened by examining immediate, contributing and underlying causes so organisations can prevent similar events from occurring again.
Participants learn how to secure an incident scene, gather and preserve evidence, conduct witness interviews, analyse human and equipment factors, determine root causes and prepare clear investigation reports with practical corrective actions.

Assessment & Evaluation
The PIMS Accident & Incident Investigation assessment evaluates understanding of investigation principles, evidence gathering, cause analysis and corrective-action development.
- The examination is open book and must be completed within 2 hours.
- A minimum score of 65% is required to pass.
- Learners who do not pass may retake the examination.
- A maximum of three examination attempts is permitted within a 48-hour period.
- After three unsuccessful attempts, learners must wait 48 hours before attempting the examination again.
- Once the examination has been passed, it cannot be retaken solely to improve the score.
